The patient safety card is designed to both provide information for patients on safety and safety planning, and help women recognize how their relationship impacts their health as well as the lives of their children. The safety card offers helpful suggestions for patients to improve their health and safety, and lists specific health problems that may be the result of chronic stress from an abusive relationship. It also offers women who are in abusive relationships guidance on talking with their children. The backside of the card refers people to the National Hotline on Domestic Violence for further support. This tool is most effective when placed in private areas and may be given to patients routinely as part of regular visits, or upon disclosure of abuse.
